Dr. Martin currently sees patients
all day Monday and Friday and Tuesday mornings. In most cases,
appointments can be scheduled within two weeks. If you have an emergency,
every effort will be made to see you on the day of your call. You are
required to send or bring past medical records, x-rays or films of any
kind.
Please bring the following items with you to appointments:
- Your
insurance card or other health coverage information.
- Shorts
containing no metal.
- Any
X-rays, MRI's, MRI Arthrograms, CT scans, Bone Scans, EMG Reports.
- Clinic
notes or operative reports from your referring physician that relate
to your condition.

constantly progress in improved treatment of hip disorders, we will be
collecting data as we go. We ask you to help us by printing and bringing
the completed forms below with you to your first appointment. If you do
not have printing capabilities, these forms will be provided for you at
your first visit.
